Upcoming Forum for Advanced Care Planning

Learn about new initiatives about Advanced Care Planning from experts in the field from across Australia.


Palliative Care WA invites carers, health consumers and all community members to take part of their forum on Advanced Care Planning (ACP), which seeks to inform the wider community about how ACP works and how they can be involved.

The forum, planned for Thursday, 25 March, from 9:30 AM to 1:00 PM, will take place at the Joondalup Resort, Country Club Boulevard, in Connolly. Participants may attend in person at this location or online.

The main goal of this forum is to increase the community understanding and engagement with ACP. The forum aims to reach out to aged people from all walks of life, including those who are well and still living in their home, those diagnosed with a chronic or life-limiting illness and those experiencing a decline in their aged care.

The forum hopes to empower aged people with information about their future care and ease the burden and stress of decision-making for family and carers.

Health professionals are also encouraged to attend. Towards the end of the program, there will be an opportunity to participate in a strategic planning activity that may help shape the future of ACP in WA.

Guest presenters
The forum will include talks from advance care planning innovators from around Australia, including Dr Charlie Corke, ACP Clinical Lead at South West Victorian Health and other experts, including two WA-based professionals. See below for the complete list of presenters:

  • Associate Professor Charlie Corke -The regional Clinical Lead for the ACP program for South West Victorian Health, Charlie is a strong advocate of patient centred care.  He is the originator of the MyValues approach to Advance Care Planning, and the author of Letting Go, LINK written to empower people to take more control of their final stage of life.
  • Dr Merran Cooper – Merran is a palliative care specialist, and founder of Touchstone Life Care, a technology company that brings Advance Care Planning into the 20th century. She is passionate about fostering compassion and better communication throughout our medical system.
  • Jeremy McKnight – As the General Manager of Shannon’s Bridge, a volunteer run charity in Melbourne, Jeremy has a personal story to share on how it was established. Shannon’s Bridge connects patients and families with supports and services when dealing with end of life issues. They also educate the community on Advance Care Planning.
  • Vicki Barry – Vicki is the Co-founder and Director of Your Departure Lounge. With a passion to ensure that we all get the best death possible, Vicki co-founded Your Departure Lounge to empower, motivate and support people to plan and prepare for their final journey. This for-purpose charity aims to develop and scale a peer-led, social impact model for end of life planning in the community.
  • Chris Jeffery – Chris is a Senior Policy Officer with COTA WA and a member of the Advance Care Planning Consortium, a forum for individuals and agencies to share strategies and encourage uptake of ACP in WA.
  • Sabena Lund – The Community Education Officer at Palliative Care WA, Sabena develops and delivers a range of community education activities. These include our highly successful ACP workshops that have been delivered in partnership with community groups from Geraldton to Esperance and all points in between.

  • $35 Sector Professionals ticket. This is for those working in the sector who are not members of PCWA.
  • $15 Community ticket. This is for members of the community, those who are unwaged, students and volunteers.
